Start Logic Forms Training Pages

Here is how to Create a Form in your websites where someone may fill in their Name and address and other information and they can submit to you and you will receive it in your email. I will now explain to you how to set up your pages using Start Logic. Here is how to set up your Start Logic Form Page to bring in Additional new members to use with your Web Pages.

1. Once you have purchase Start Logic you need to go to this site to start your Forms page.

2. Once you get to this site enter your User name and Password that was sent to you when you applied for Start Logic and click on Login !.

3. Once you enter your site click on PowerPlugins.

4. When the next page comes up click on Form Mail.


5. Another page will come up and this is what it will say.


.Choose the action you wish to perform. If you want to redesign the form, view and save the raw HTML and edit in your favorite editor. Be careful not to remove any of the hidden fields, as these are required for input validation.







New form name:

Club Bucks Form and click on Add New Form.


6. Here you may want to add in the Enter Form Name Here.

7. Then this is what will show up.

8. When this page shows up click on the Text Down Arrow and Select Text Area

9. Type in  the Input name on the right side of New  (Comments & Questions)

10. In the Values space opposite New Type in:(Yes, Mail Me a FREE Club Bucks Card Now!)

11. Then Click on: (Update Form Inputs)

12. This is what will come up.

13. Now the Basis for your Form is completed. The rest is just simply adding in the fields that you want to add and Clicking on the Update Form Input.

14. Some of the common fields that you may want to add are, one at a time.

     A. First and Last Name.   ( or you can have these each on a separate line)

     B. Email Address

     C. Confirm Email address

     D. PO Box or Street Address

     E. City

     F. State

     G. Zip Code

     H. Area Code only

      I. Tel Number only

15. If you make a mistake on any of them you can check mark them and Click on the Update Form Input and it will delete them or you can delete the whole form and start over again.  

16. My end result looked like this below. Plus I added in the text to my page that is marked in red below. You can put what you want there above your Comments or Questions Text..........................

17. To see how it looks you can click on the Fields and you will see what your form will look like.

18. The next step once your complete form is completed is to Click on the Link in the bottom of the page that says Edit Submission Email.

19. This is simular to what will show up in yours with the exception that instead of what I have in my white text area below, you will have.

Enter template. Use [% field_name %] to mark where to put data.

Define what happens when the form is submitted. The form may only be sent 'To' one e-mail address, but can be Cc'd and/or Bcc'd to up to 5 other addresses. If Cc/Bccing form to multiple addresses, delimit addresses with commas.

20. Here when you get to this page you will see and want to highlight the:  Enter template. Use [% field_name %] to mark where to put data.  that is in your white box and delete it.

21. Now to make your form work properly, you need to highlight the data in the bottom under Available Fields on your page and not necessarily this page material.  Click on Copy.

My example is this.Yours will be simular depending on which fields you select.: ( [% Form_Submit %] [% Telephone_Number_only %] [% Area_Code_only %] [% Zip %] [% State %] [% City %] [% PO_Box_or_St_Address %] [% Email__Address %] [% First_and_Last_Name %] [% Comments_or_Questions__ %])

    and paste it in the white box where you deleted the above information.

22. Next you must enter the Email address that you want this form material to be sent to. If you look at the top I also included a CC Email address as a backup.

23. When you complete adding the above information your Edit Submission Email should look like this below with the exception of your email address instead of mine and the fields that you chose to pick in the white text area.

 Now click on: Update Email Details and your information will be saved.

24. The next step is a very important step and that is to insert your Form html code into your page or automatically add it in following the instructions under How to use.  It contains this material.

Our servers use the XBitHack Apache directive to identify SSI pages. So, for this to work, you must check that the page is chmoded 754.

If you want to completely edit the design of the form, you can also amend the raw HTML and insert that into your page manually.


25. To Find the File Manager in start logic simply look on the left side  of your page where you are creating this form and Click on the Host Manager and the File manager will come up.

26. Click on File Manager and  when it comes up, if you have already uploaded the page that you want your form to be placed on and they are showing then go on to Number 30 below. If you can see Public.html click on that and your pages may come up. If you have not uploaded your pages yet then go on to Number 27 below.

27. Here you can upload your page using the Browse on the bottom of the page. By clicking on the Brows this will take you into your harddrive to get the page you want to upload.

28. This page should be written ahead of time  using either Microsoft  Word or your Publishing Program and SAVED AS   an html document  file rather than as a Doc File or a TEXT file.    (Of course you need to be awared of what the name of the file is and where it is located on your hard drive before you BROWSE For it.).  Be sure you are looking for it with your    HTML DOC or your ALL FILES showing in the bottom of your OPEN Window rather than a Doc file or a Text File.. showing. If you have saved it and you can't find it using the browse button then go back to your hard drive and click on the start button and click on Search Files and Folders until you find it. NOTE where it is and you can come back to Browse in Start logic's File manager and find it.

 29. If you have not already uploaded it now you can upload it to your Public.html folder or the one showing here with other files.

 30. Once you find the page that you want your Form to be placed on slide over to the Right side  of the name and click on the Little Pencil. This will allow you to Edit the page by bringing up your page in the File Managers Page Editor.  (This is an excellent Publishing Program that can be used to Create your pages for yourself.)

 31. Slide over to the very far right side of the Page Editor all the way on the top of the Tool Bar and click on the LITTLE ICON that says Insert Object when you place your cursor over it. Click on it and Select Object to Insert windo will come up.

 32.  If you have more than one Form to pick from then click on the Down Arrow and select the one you want and Click on Insert.

 33. This will automatically insert your form on the page that you have selected. Then you must Save Changes at the bottom of your page.

 34. Or, as the HOW To USE instructions page in Start Logic Forms indicates,

 if you want to edit your page manually, from your own publishing program you can add a form by placing this code on your page in the html part of the page.   <!--#include virtual="/v-web/forms/285"-->    OR  If you want to completely edit the design of the form, you can also amend the raw HTML and insert that into your page manually.

 35. We now have one more thing to do as laid out in the HOW To USE Instructions page It says.

Our servers use the XBitHack Apache directive to identify SSI pages. So, for this to work, you must check that the page is chmoded 754.

 36. You will notice in your File Manager page where all of your pages are listed on the Far Right hand side are a series of 3 numbers for most of your pages.

 37. To make your page work you must change the number that is there to    754.   This will allow someone to write on your website forms page and submit it to you. Without this permission code no one will be able to Submit their information and have it Emailed to the Email address that you requested that it be sent to above.

 38. Simply Click on the number that is there, it may be a 644 now and a box will come up. Type in 754 and Click OK.

 39. You may then have to Click again on Public.html  and your page will come back showing that it now has 754  as the new permission code.

 40. Walla You are done.

 41. If you manually add the Forms HTML code to your pages manually and you are using WSFTP software to upload your pages or some other form of uploading you will need to log into Stat Logic at      http://www.(your domain name)/vdeck

 42. Add your User name and Password that they provided you with and Click on Host Manager and then on File Manager to Change the Permission Code as explained above.

 43.  Now you can bring up your browser and Type in your     http://www.(your domain name)/(Your file name of the page where your form is on).html

44. No spaces in your file name. Mine is:      http://www.rolanddufault.com/MoreTesting.html
